New Online System from 1st October

From 1 October, all requests made by phone, in person or online will go through Patchs Health, our new system.

Long term conditions

If you have a long term condition (also called a chronic illness or chronic disease) then it is essential that you have regular health checks. Depending on your condition it may be an annual check, a six-monthly check or more frequently.

The aim of the health check is to keep you healthy and to reduce the impact of your illness on your life. We will contact you when your check is due, but please contact if you are unsure when your next health check is due or if you have any concerns about your long term condition.

Many long term conditions need regular monitoring to ensure you don’t become unwell. This is important for your health, so please do book an appointment when we ask you.
