New Online System from 1st October

From 1 October, all requests made by phone, in person or online will go through Patchs Health, our new system.

Changing your contact details

To update details like your name, address, phone numbers or email address:

Change of address

If you have moved to a new address you need to let us know. This is because we need to make sure you are still in our catchment area and can stay registered with the surgery. We also will need to send you letters about your health from time to time, so it’s important we send this to the correct address.

Changing your phone number or email address

You will need to show us identification before we can change your phone number or email address.

Changing your name

You will need to show us proof that you have changed your name. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
