Structured medication reviews

A structured medication review (SMR), sometimes known as a medicines review, is an appointment between you and a Pharmacist to discuss your health and your medicines. The review will last approximately 20 minutes, but please let us know if you need more time.

This review aims to make sure that you are being prescribed the best medicines for you, and that you get the most safe and effective treatment available.

Why do I need a medicines review?

A medicines review is there to make sure your medications are safe and working well for you. This is more important for people who take lots of different medicines, or people with lots of medical conditions, but anyone on any regular medications may need a review.

If you are due a medicines review, we will contact you either by text message or by letter to let you know. It is important to have a medicines review if you are due one. Not completing a review may mean that your repeat prescription is delayed, or that you are given a smaller supply of medication until the review has been completed.

If you are a new patient to the practice and take regular medicines, you may be invited as part of your registration checks to ensure the your medication records are up to date.

What happens at the review?

You will have an appointment with one of the practice pharmacists.  They work with your usual GP and will ask questions to see how you are managing with your medicines. This review is also your chance to tell us more about how your medicines work for you, and gives you the opportunity to ask any questions you may have about your medications.

Your wishes are important to us. No medicines will be changed without your agreement and if needed, the Pharmacist will arrange any follow up appointments with you.

This review can normally be done by telephone, but can be done face-to-face if needed.

How should I prepare for the review?

It is good to have all your medicines with you when you have your appointment. If someone else helps you to manage your medicines, you may want to ask them to be at the appointment with you.
