Child new patient registration

Use this service to register your child with the surgery.

You can use this service if you:

• want to register at the surgery
• live within our catchment area
• are registering a child aged 0 to 15

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• the details of the child you are completing the form on behalf of
• your child’s NHS number
• details of previous GP surgery
• basic health and medical information
